Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities, under the Clinical Manager. You will develop and review plans of treatment, assess speech, language, voice and swallowing disorders, and provide counseling to patients and families.

Ideal candidates hold ASHA CCC, an active SLP license and CPR certification with reliable transportation; home health experience is preferred.

Qualifications

Certification of Clinical Competence in Speech-Language Pathology (ASHA).
Active state license to practice speech-language pathology.
Current CPR certification.
Driver's license and reliable transportation.

Responsibilities

Evaluate and provide speech-language pathology services in home or facility settings.
Contribute to Plan of Treatment and Plan of Care development and review.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer & interpret diagnostic tests and therapy applications.
Observe and report changes in patient condition to supervisor or physician.
Provide training on alternative communication systems when appropriate.
Counsel patients, families, and healthcare staff; document visits and care plans.
Participate in care coordination and discharge planning.
