Job summary

Commonwealth Home Health, part of the LHC Group and within the Optum family, seeks a dedicated Physical Therapist Assistant to deliver therapy and education in patients' homes across Fayette and Jessamine counties. The role supports a plan of care designed by a supervising Physical Therapist.

You will instruct active and passive exercises, assist with gait and mobility, document responses, and coordinate with the healthcare team.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Follow state-specific laws governing home health PT; work only per supervising PT’s plan of care.
• Instruct and aid patients in active/passive exercise, gait, ADL, transfers, and safety.
• Observe, record, and report patient responses; coordinate care with the health care team.
