Job summary

IHSHawaii is seeking a Family Case Manager to support families experiencing homelessness by building life skills and paths toward long-term independence. You will conduct intakes, assessments, and collaborative service plans with a trauma-informed, culturally sensitive approach.

You will coordinate with community resources, provide coaching and counseling, document progress, and respond to crises while upholding dignity and respect in every interaction. This is a full-time role.

Qualifications

• High School Diploma or GED required.
• Minimum 2 years of direct case management or related experience.
• Knowledge of issues impacting formerly homeless or at-risk families.
• CPI certification (or ability to obtain), CPR and First Aid certification.
• Valid Driver’s License with a clean driving record.

Responsibilities

• Serve as the primary support and advocate for families
• Conduct intakes and holistic assessments with empathy and professionalism
• Co-create strengths-based service plans and support progress toward goals
• Provide hands-on coaching, problem-solving, and counseling
• Respond to crises with calm, trauma-informed approach
• Connect families to community resources and ensure engagement
• Maintain accurate, timely documentation reflecting progress
