Responsibilities

  • Own a caseload and guide clients through assessment, service planning, and referrals — centering their voice and choice.
  • Connect people to housing services, substance abuse and mental health treatment, transportation, and coordinated care.
  • Show up in crisis moments with risk assessments, crisis plans, and hospital referrals as needed.
  • Keep meticulous, compliant records that protect clients and strengthen programs.
  • Drive to appointments and meet people where they are to support their trajectory.
