Qualifications

High school diploma or GED required; college degree a plus.
1–2 years case management or supportive services experience with homeless/underserved populations is a plus.
Knowledge of homelessness, mental health, and substance abuse issues.
Strong time-management and organizational skills.
Excellent communication and conflict-resolution abilities.

Responsibilities

Own a caseload and guide clients through assessment, service planning, and referrals.
Connect clients to housing, transportation, substance abuse and mental health treatment with partner providers.
Respond in crises with risk assessments, crisis plans, and hospital linkages.
Maintain meticulous, compliant client records.
Provide outreach and transportation to appointments as needed to change trajectories.
