Job summary

The Institute for Human Services, Inc. in Honolulu is seeking a Case Manager to support individuals and families facing homelessness. You’ll assess needs, plan services, and connect clients to housing, health care, and transportation with a strong focus on client voice and choice.

This full-time role offers training, mentorship, and clear pathways for advancement. You’ll work in crisis moments, maintain accurate records, and collaborate with clinicians, housing specialists, and partner providers

Qualifications

• High school diploma or GED required; college degree a plus.
• 1–2 years case management or supportive services experience with homeless/underserved populations is a plus.
• Knowledge of homelessness, mental health, and substance abuse issues.
• Strong time-management and organizational skills.
• Excellent communication and conflict-resolution abilities.

Responsibilities

• Own a caseload and guide clients through assessment, service planning, and referrals.
• Connect clients to housing, transportation, substance abuse and mental health treatment with partner providers.
• Respond in crises with risk assessments, crisis plans, and hospital linkages.
• Maintain meticulous, compliant client records.
• Provide outreach and transportation to appointments as needed to change trajectories.

Expectations

As a Case Manager, You'll

• Own a caseload and guide clients through assessment, service planning, and referrals — always centering their voice and choice in the process


• Connect people to what they need — substance abuse and mental health treatment, transportation, and coordinated care with partner providers


• Show up in crisis moments — conducting risk assessments, coordinating crisis plans with psychiatric providers, and linking clients to hospitals and crisis teams when it matters most


• Keep meticulous, compliant records that protect your clients and strengthen our programs


• Drive shared efforts, meeting people where they are — sometimes literally, behind the wheel, getting them to the appointments that change their trajectory

What You Bring

• A high school diploma or GED (bonus points for an Associate's or Bachelor's degree)


• Willingness to learn and train but 1–2 years of case management or supportive services experience with homeless or underserved populations is a plus


• A working knowledge of homelessness, mental health, and substance abuse issues


• Sharp time-management skills


• Communication skills built for conflict resolution


• Integrity, patience, and a genuine commitment to the mission


• Comfort with ambiguity and a fast-moving environment


• A valid driver's license and a clean traffic record
