IHS Hawaii in Honolulu is seeking dedicated Case Managers to support individuals facing homelessness through assessment, planning, and referrals. You will guide clients to needed services, maintain careful documentation, and work with a team to ensure coordinated care.

This role emphasizes client voice, trauma-informed practice, and community connection. What you bring includes a high school diploma or GED (college degree a plus), 1–2 years in case management, and a valid driver's license.

Qualifications

Responsibilities

  • Connect people to housing services, substance abuse and mental health treatment, transportation, and coordinated care.
  • Show up in crisis moments with risk assessments, crisis plans, and hospital referrals as needed.
  • Keep meticulous, compliant records that protect clients and strengthen programs.
  • Drive to appointments and meet people where they are to support their trajectory.

Expectations

As a Case Manager, you'll:

  • Own a caseload and guide clients through assessment, service planning, and referrals — always centering their voice and choice in the process
  • Connect people to what they need — substance abuse and mental health treatment, transportation, and coordinated care with partner providers
  • Show up in crisis moments — conducting risk assessments, coordinating crisis plans with psychiatric providers, and linking clients to hospitals and crisis teams when it matters most
  • Keep meticulous, compliant records that protect your clients and strengthen our programs
  • Drive shared efforts, meeting people where they are — sometimes literally, behind the wheel, getting them to the appointments that change their trajectory
What You Bring
  • A high school diploma or GED (bonus points for an Associate's or Bachelor's degree)
  • Willingness to learn and train but 1-2 years of case management or supportive services experience with homeless or underserved populations is a plus
  • A working knowledge of homelessness, mental health, and substance abuse issues
  • Sharp time-management skills
  • Communication skills built for conflict resolution
  • Integrity, patience, and a genuine commitment to the mission
  • Comfort with ambiguity and a fast-moving environment
  • A valid driver's license and a clean traffic record
