Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Thompson, PA to provide hands-on, person-centered support to individuals with IDD in home and community settings.

The DSP will assist with daily living activities, nutrition, mobility, and medication as authorized, while implementing behavior support plans and promoting independence. A high school diploma is required, with EH support and regulatory compliance emphasized.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support preferred.
• English proficiency required.
• Valid driver’s license may be required.

Responsibilities

• Provide hands-on, person-centered support in home and community settings.
• Assist with daily living activities, meals, mobility, and medications as authorized.
• Follow ISP/IPP care plans and document observations and outcomes.
• Maintain confidentiality and comply with HIPAA and regulatory standards.
