Job summary

AHRC Nassau seeks a Direct Support Professional to provide guidance and support to individuals in the Community Living Services program, promoting independence, health, safety, and a high quality of life in daily routines.

You will assist with self-care, meals, medication administration, transportation, and community participation, while documenting plans and working with an interdisciplinary team to achieve valued outcomes.

Qualifications

• High School Diploma or equivalent preferred.
• Must have NYS driver’s license and maintain qualified driver status.
• Ability to communicate and understand written and oral directives.

Responsibilities

• Provide ongoing guidance and support to individuals to ensure active participation in daily life and independence.
• Support community inclusion and build natural supports and relationships.
• Maintain a safe, clean home and assist with household chores as needed.
• Follow treatment plans and use adaptive/safety equipment to promote wellbeing.
• Assist with meal preparation, diet adherence and cooking skills.
• Assist with medication administration per AMAP policy.
• Document treatment plans, medical notes, and progress toward goals.
• Adhere to scheduling, overtime, and staffing policies to ensure daily coverage.
• Participate in interdisciplinary team meetings and share information.
• Provide transportation for medical appointments, shopping, and activities.
• Mentor new Direct Support Staff and review training materials.
