AHRC Nassau is seeking a Direct Support Professional to help people in Community Living Services live independently and safely. You will provide daily living supports, assist with self-care, health, safety, and social activities, and help build meaningful community connections.

The role includes supporting meal prep, facilitating medication administration per AMAP, and documenting progress in care plans. Regular team meetings and driving individuals to appointments are part of the duties.

Competenze

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver’s license and maintain qualified driver status.
• Must be able to communicate and understand written and oral directives/information.

Mansioni

• Provide ongoing guidance and support to individuals with disabilities to promote independence and engagement in daily routines.
• Assist individuals with self-care routines, health, safety, household chores, socialization and overall wellbeing.
• Support individuals in meal preparation and dietary adherence.
• Administer medications according to AMAP policy and procedures.
• Document and implement treatment plans and agency materials to support independence and wellbeing.
• Attend inter-disciplinary team meetings and contribute to progress discussions.
• Provide transportation to medical appointments, shopping, recreation, and other activities.
• Mentor new staff by modeling supportive routines and reviewing training materials.
