Job summary

Qualifications

• High school diploma required.
• Experience working with individuals with mental health diagnoses, intellectual/developmental disabilities, or substance use disorders is preferred.
• Driver's license (valid) required in USA.

Responsibilities

• Assess individuals on hopes, dreams, and desires; provide supports to achieve them.
• Help individuals build community relationships and utilize natural supports.
• Assist with daily living activities and community participation aligned to interests.
• Ensure rights are protected and promote advocacy and self-determination.
• Complete documentation, progress notes, and incident reports as required.
