Job summary

Qualifications

• High School Diploma required.
• Valid driver's license (USA).
• Experience with individuals with mental health diagnoses or developmental disabilities or substance use disorder preferred, not required.

Responsibilities

• Assess each person’s hopes and provide supports to achieve them.
• Help people develop relationships in their community and with natural supports.
• Assist participation in the community in line with interests.
• Protect and promote rights and advocate for individuals as needed.
• Provide emotional, physical, and medical support as required.
• Foster person-centered, positive relationships to promote self-determination.
• Contribute to assessment and planning and implement the plan.
• Complete documentation including goals and progress.
• Complete daily progress notes and logs to track appointments and goals.
• Assist with daily living tasks (home duties, meals, laundry, shopping, etc.).
• Arrange or provide transportation as needed.
• Support safety procedures and emergency knowledge.
• Mentor and train staff as needed.
• Sleepover may be required.
• Attend meetings and maintain required trainings.
• Follow policies and state/federal regulations.
• Perform other duties as assigned by supervisor.
