Qualifications

• Bachelor's degree in a human services field OR a Bachelor's in a non-human services field with two years of human services experience.

Responsibilities

• Advocate for individuals and educate them and their families.
• Collect information and determine eligibility for services.
• Plan and update Person-Centered Plans (PCPs) for service delivery.
• Coordinate resources and referrals to meet individual goals.
• Monitor PCPs and ensure compliance with standards.
• Collaborate with leadership and teams to maintain quality service.
