Qualifications
- Bachelor's degree in a human services field OR a Bachelor's in a non-human services field with two years of human services experience.
Responsibilities
- Advocate for individuals and educate them and their families.
- Collect information and determine eligibility for services.
- Plan and update Person-Centered Plans (PCPs) for service delivery.
- Coordinate resources and referrals to meet individual goals.
- Monitor PCPs and ensure compliance with standards.
- Collaborate with leadership and teams to maintain quality service.
