Job summary

Service Coordination, Inc. (SCI) is seeking Service Coordinators to provide community-based case management for individuals with disabilities in Hagerstown, MD.

The role emphasizes advocacy, education, and coordination of supports to help people achieve independence. We offer a competitive base salary of $64,395–$79,861, travel reimbursement, a quarterly bonus potential, generous benefits (medical, dental, vision), life insurance, 403(b) match, tuition reimbursement, and extensive training.

Qualifications

• Bachelor's degree in a human services field or related field.
• Experience with people with developmental disabilities is preferred.
• Knowledge of person-centered planning and DDA services is preferred.

Responsibilities

• Provide Case Management services to individuals with intellectual and/or developmental disabilities.
• Establish and monitor Person-Centered Plans (PCPs) and annual outcomes.
• Identify and secure ongoing support services and coordinate with external stakeholders.
• Complete required paperwork and ensure compliance with state standards.
