Job summary

Lower Shore Enterprises, Inc. is seeking a Case Manager to oversee a caseload of participants in our DDA/DORS programs.

You will develop and implement employment goals based on person-centered planning and promote community participation through work, volunteering, and social activities. You will train DSPs, monitor PCP progress, and collaborate with program directors to build skills for competitive employment.

Qualifications

• Bachelor’s degree or equivalent experience in case management required.
• Must pass a criminal background check and have a clean driving record.
• CESP certification within one year of hire.
• Certified Med Tech within 6 months of hire.
• Proficiency with Windows applications, particularly Word and Excel.

Responsibilities

• Manage a caseload of participants receiving DDA and DORS services.
• Create and implement person-centered employment goals.
• Promote community employment through discovery and day services.
• Train Direct Support Professionals on participant PCPs.
• Document PCP progress and related schedules; ensure billing per regulations.
• Interact with the business community to secure employment opportunities.

Education

Bachelor’s degree or equivalent experience in case management

CESP certification within one year of hire

Certified Med Tech within 6 months of hire
