Resources for Human Development (RHD) in Delaware is seeking a compassionate Case Manager to join a clinical, interdisciplinary team. You will coordinate residential, psychosocial rehabilitation, and personal care services to help individuals achieve greater independence and well-being.

The role requires a Bachelor’s Degree and at least 5 years of direct experience in disability services, with 2 years of supervisory experience. Part-time position with DailyPay and benefits availability.

Formación

• Bachelor’s Degree required; 5+ years in disability services.
• 2+ years supervisory experience required.
• Valid Delaware Driver’s License and 3+ years driving history.
• Strong written and verbal communication; organized and proactive.
• Ability to collaborate with internal and external partners.

Responsabilidades

• Case Management & Service Coordination: assess needs, coordinate services, and work with clinical staff and families.
• Daily Living & Independence: support activities of daily living and independence skills.
• Health, Safety & Medication Support: monitor health and safety and assist with medications.
• Documentation & Compliance: complete documentation, protect confidentiality, and follow HIPAA policies.
