Qualifikationen

• Minimum of 1 year of successful experience working with individuals who have disabilities, providing case coordination/case management, or serving in a quality assurance or compliance capacity.
• High school diploma or equivalentrequired; Bachelor's degree in a related field preferred.
• Excellent written and oral communication skills.
• Highly organized and demonstrated ability to work well independently and as part of a team
• Experience in using a client management system. iCare Manager preferred.

Aufgaben

• Provide case coordination services to participants in OBI programming by ensuring meaningful programming and high quality care through the PCP process.
• Collaborate with Coordinators of Community Services to facilitate timely PCP meetings.
• Work with each participant and their team to establish person-centered goals in the SIP.
• Maintain close communication with OBI direct support professionals to assist with planning and progress.
• Develop and submit all required documentation to request and secure DDA funding for each participant.
• Monitor participant documentation within OBI’s online database for accuracy and up-to-date progress.
• Involve OBI’s Workforce Development and Manager, Personal Supports as needed.
• Review and authorize services and documents for PCP and LTSS as required.
