Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Experience in an acute care hospital setting preferred.

Responsibilities

• Assess and plan patient care and discharge needs with input from the health team.
• Implement the plan of care through direct patient care, coordination and supervision.
• Evaluate patient and family response to care and revise the plan as needed.
• Document care in the patient record with timely and accurate entries.
