Qualifications

• Must possess a current RN license in the state of practice.

• Experience in professional nursing practice and patient education.

Responsibilities

• Assess patient needs across physical, psychological, social, and educational dimensions.
• Formulate and revise a plan of care with the health team.
• Coordinate and supervise care delivery with licensed staff.
• Document care and communicate patient status to the care team.
• Contribute to patient outcomes through evidence-based practice.
• Support education for patients and families.
