Job summary

GBMC HealthCare is seeking a Social Worker in Care Management to assess and intervene for patients transitioning from the hospital, with supervision of the ED Discharge Coordinator team. The role focuses on crisis and behavioral health, coordination with ED, physicians, and community resources to shorten ED stays and ensure safe transitions.

The ideal candidate holds a Master’s in Social Work with 1–3 years in hospital discharge planning or crisis care, and licensure by Maryland social work

Qualifications

• Master’s in Social Work or equivalent experience.
• 1–3 years hospital social work or discharge planning experience.
• Licensed in Maryland as LMSW, LCSW or LCSW-C.

Responsibilities

• Supervise the ED Discharge Coordinator team and coordinate with ED staff and Care Management.
• Assess psychosocial needs and develop comprehensive care transition plans for patients.
• Interview patients and families, educate on resources, advance directives and post-discharge options.
• Coordinate with community resources and post-acute services to facilitate timely discharges.
