Job summary

Qualifications

• Bachelor's degree required or must be completed within 3 years of hire.
• Three years’ progressively responsible work experience in psychiatric/chemical dependency field or related health care setting.
• Experience in discharge planning and ED processes.

Responsibilities

• Gathers and evaluates information to prepare patient discharge.
• Meet with patients and families in the ED to obtain psycho-social history relevant to discharge planning.
• Communicates with the treatment team to address social, emotional or environmental issues.
• Collects necessary documents ( Advanced directives, MOLST, custody agreements, etc.).
• Maintains and updates electronic medical records with discharge information.
• Organizes discharge packets for transfers to inpatient facilities.
• Works with crisis beds and protection program liaisons as needed.
