Responsibilities

• Supervise the ED Discharge Coordinator team and collaborate with ED Psychiatry, ED Providers and ED Nursing.
• Conduct psychosocial assessments and develop patient care transition plans.
• Coordinate post-discharge resources with families and community agencies to reduce ED length of stay.
• Provide counseling and motivational interviewing; address end-of-life care, advance directives and health care navigation.
• Document all pertinent information and participate in department initiatives related to process improvement.
