Job summary

GBMC HealthCare seeks a discharge planning professional to coordinate patient discharge from the Emergency Department. You will work with a multidisciplinary treatment team to assess needs and discuss placement options with patients and families, ensuring timely and compliant discharge planning.

Responsibilities include gathering psychosocial history, obtaining records, updating the care team, and organizing documentation for post-discharge care while adhering to HIPAA and patient privacy

Qualifications

• Bachelor's degree from an accredited institution is required.
• Must be working towards a bachelor's degree and must be completed within 3 years of hire.
• Three years’ progressively responsible work experience in psychiatric/chemical dependency field or related health care setting.

Responsibilities

• Gathers and evaluates information to prepare for patient discharge.
• Meets with patients and families in the Emergency Department to obtain psychosocial history.
• Contacts ED personnel and/or retrieves medical records to plan discharge needs.
• Communicates with the treatment team about discharge planning barriers or updates.
• Prepares and maintains electronic medical records and exchanges documentation with outside facilities.
• Assists in securing appropriate placements and arranging bed availability for discharge.
