Job summary

The position requires a bachelor’s degree (or progress toward it within 3 years) and at least three years of relevant psychiatric/healthcare experience.

Qualifications

• Bachelor's degree is required or must be working towards it and completed within 3 years of hire.
• Three years' progressively responsible work experience in psychiatric/chemical dependency field or related health care setting.

Responsibilities

• Gather and evaluate information to prepare discharge by meeting with patients and families.
• Coordinate with ED personnel and obtain records to meet discharge needs.
• Communicate with treatment team to exchange information on social, emotional or environmental issues.
• Collect and file advance directives, MOLSTs, custody agreements and other documents affecting discharge planning.
• Educate patients and families about the discharge process and obtain consent signatures.
• Provide bed availability and disposition updates, including special cases if patient is a minor.
• Prepare and maintain electronic medical records and ensure sharing with outside facilities when appropriate.
• Agree on discharge plans with crisis beds and related programs as needed.
• Assess needs for home care or extended care facility placements and secure psychiatric facility placements as recommended.
