Job summary

• AVADE training within 30 days of hire.

Responsibilities

• Performs intake assessment/evaluation of clients and families.
• Participates in multidisciplinary treatment planning.
• Assesses discharge planning needs and alternative placement arrangements.
• Petitions guardianship/conservatorship for eligible adults.
• Documents evaluations and recommendations in the medical record.
• Assesses for signs of abuse and coordinates referrals.
