Job summary

Greater Baltimore Medical Center (GBMC) seeks a Social Worker to support Care Management, supervise the ED Discharge Coordinator team, and coordinate patient transitions from the Emergency Department to post-acute care. Qualifications

• Master's in Social Work required; equivalent experience or certification considered.
• 1–3 years hospital or health care discharge planning, crisis and behavioral health experience preferred.
• Maryland LMSW/LCSW/LCSW-C licensure required.
• Experience interviewing patients and families, assessing psychosocial needs, and developing care plans.
• Ability to work in a fast-paced, multidisciplinary hospital setting and engage with diverse populations.

Responsibilities

• Supervise the ED Discharge Coordinator team and coordinate with ED Psychiatry, physicians, and nursing.
• Guide caseload management, prioritization, and problem-solving for complex cases.
• Liaise between Care Management and ED to support timely discharges.
• Conduct psychosocial assessments with patients and families to inform transition planning.
• Develop and implement comprehensive care plans and coordinate post-discharge referrals and community resources.
• Document all assessments and interventions timely and participate in ongoing process improvements.
