Job summary

GBMC HealthCare in Baltimore seeks a Social Worker in Care Management to assess and intervene for post-discharge continuity, supervise the ED Discharge Coordinator team, and collaborate with ED physicians, nurses, SBIRT Peer Recovery Coaches, and safety programs. This role will also coordinate with the Care Management team to ensure timely discharges and patient safety.

Qualifications

• MSW degree and Maryland LMSW/LCSW licensure required.
• 1–3 years hospital discharge planning or crisis behavioral health experience.
• Strong communication with patients, families, and care teams.
• Ability to work with diverse populations and in fast-paced settings.

Responsibilities

• Supervise the ED Discharge Coordinator team and coordinate with ED providers.
• Perform psychosocial assessments and develop care transition plans.
• Facilitate family meetings and connect patients with community resources.
• Document findings and participate in process improvement initiatives.
• Collaborate with hospital and community agencies for safe discharges.
