Job summary

Care Lync is seeking a full-time Case Manager to coordinate support services for Nevada participants enrolled in Medicaid waivers. The role requires strong organization and attention to detail to assess participant needs and develop individualized plans.

You will coordinate physical, behavioral, and support services with families and other providers to improve independence. The position focuses on meeting program requirements, maintaining training and certifications, and ensuring compliant care

Qualifications

BS/BA degree in related course work.
No prior experience required.
Experience with Microsoft Office, state-maintained databases.
Must maintain proficiency in company sponsored training and certifications.
Maintain updated state related Clearances.
Successfully complete and maintain training courses as required or amended by program regulations.
Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration, copies must be maintained on file with Care Lync.

Responsibilities

Discerning the disabilities and accompanied needs of participants and helping them achieve their goals.
Make a plan for each participant and modify it annually or when there is a significant medical or social change.
Communicate with other individuals and entities to ensure seamless coordination between physical, behavioral, and support services.
Develop and maintain professional working relationships with participants and their families.
Fulfill the mission of providing support coordination and case management services in Nevada.
