Job summary

Wise & Healthy Aging seeks a Care Coordinator for its Elder Abuse Prevention Program in the Long Beach area. You will conduct client intakes, in-home assessments, and develop care plans with measurable outcomes while coordinating services and monitoring progress.

Travel to client homes is required, with strong documentation and communication to ensure high-quality outcomes. Qualifications include a bachelor’s degree in social work, gerontology, human services, or related field; MSW or

Qualifications

• Bachelor's degree in social work, gerontology, human services or related field; MSW or Gerontology preferred.
• 2+ years in gerontology, health care, or social services preferred.
• Fluency in English; bilingual Spanish strongly preferred.
• Valid driver’s license; able to travel in Los Angeles County/City.

Responsibilities

• Conduct client intakes and in-home assessments; develop care plans with measurable outcomes.
• Coordinate and arrange services; monitor client progress and update plans as needed.
• Maintain documentation, billing records, and program data; prepare for staff meetings; assist with outreach.
