Institute on Aging seeks a Care Manager I to provide comprehensive case management for older adults and disabled adults under the Community Living Fund. The role includes assessments, care planning, home visits, coordination of services, and ongoing client support.

• B.A. or B.S. in Social Work or related field is required.
• Experience with disabled adults and older adults is expected.
• Bilingual English/Spanish required; strong communication skills essential.

Responsabilidades

• Conduct comprehensive assessments and ongoing reassessments of clients.
• Develop and initiate the Community Living Plan with client input.
• Perform home visits and accompany clients to medical appointments as needed.
• Coordinate community services and monitor service delivery for timeliness.
• Maintain case notes, plan updates, and required paperwork.
• Engage in data collection and quality improvement activities.
• Communicate with physicians, social workers, and care providers.
