Patriot-Enterprises-LL seeks a Systems Navigator for the Exceptional Family Member Program (EFMP). You will coordinate daily care, conduct intake interviews, assess Family needs, and link Families with military and civilian services.

You will visit Soldiers and Families at home, collaborate with agencies, maintain a resource catalog, and lead monthly workshops. Strong case management, travel readiness, and Life Coach credentials are essential to support family resilience and informed care

Formación

• Knowledge of human service principles and practices and ability to connect Families with needed services.
• Knowledge of military, state and federal laws and policies related to medical, educational, and Family support services.
• Non-clinical case management skills to facilitate links between Families and required care systems.
• Strong communication skills and ability to work with diverse stakeholders.
• Level 1 Life Coach certification (Provided by Patriot).
• Must be able to travel.
• Experience with Microsoft Office.

Responsabilidades

• Provide home visits to Soldiers and/or Family members to offer support as needed.
• Coordinate with agencies and providers to identify respite care resources.
• Maintain an information catalog of networks and contacts.
• Coordinate a support group and/or workshops monthly.
• Present EFMP Systems Navigation briefings to overview EFMP services.
• Provide information and referrals to Families for appropriate resources.
• Partner with Families to build resilience and self-advocacy skills.
• Manage Soldier and Family member records per Army/DoD regulations.
• Facilitate warm hand-offs to program personnel during transitions.
• Collect data for planning, reporting, and monthly program updates.
• Implement policies that connect Families to support services.
