Job summary

Patriot Enterprises LLC is seeking a Systems Navigator for the EFMP to coordinate support for Families with special needs at Fort Bragg. You will conduct intake, assess needs, refer to services, and advocate within military and civilian networks.

The role requires a Bachelor's degree in social work or related field, two years' family case management experience, and a Level 1 Life Coach certification. Travel and background checks are required.

Qualifications

• Bachelor's degree in social work or related field and two years of experience in family-related case management.
• Must be legally authorized to work in the United States and able to pass a background check.
• Level 1 Life Coach certification required.
• Must be able to travel.
• Knowledge of military, state and federal laws related to medical and family support services.

Responsibilities

• Provide home visits to support Soldiers and Family members as needed.
• Coordinate with Government agencies and civilian providers for services.
• Maintain records per Army and DoD regulations.
• Facilitate warm handoffs to appropriate program personnel during transitions.
• Collect data for planning, reporting, and program improvement monthly.
• Implement policies connecting Families to support services.
