Job summary

Patriot Enterprises LLC seeks a Systems Navigator to support the Exceptional Family Member Program (EFMP) at Fort Benning. The role coordinates care, referrals, and advocacy for Soldiers and Families with special needs, ensuring access to medical, educational, and support services.

Responsibilities include intake, data collection, and collaboration with military and civilian providers, maintaining records per DoD regulations, and leading monthly workshops to improve program outcomes.

Qualifications

• Knowledge of human service principles and practices and understanding of family needs in case management.
• Knowledge of military, state and federal laws, policies and practices regarding medical, educational, and family support services.
• Non-clinical case management experience connecting Families with needed care.
• Effective communication skills in person and over the phone.
• Life Coach Level 1 certification (Provided by Patriot)
• Ability to travel.
• Experience with Microsoft Office programs.
• Must be legally authorized to work in the United States.
• Must pass a background check.

Responsibilities

• Provide home visits for Soldiers and Families to offer support as needed.
• Coordinate with government agencies and civilian providers to identify respite care resources.
• Maintain an information catalog of national, state, and local networks and contacts for families with special needs.
• Coordinate a monthly support group or workshops.
• Present EFMP Systems Navigation briefings to overview available EFMP services.
• Provide information and referrals to Families to help access services.
• Partner with Families to increase resilience and self-advocacy.
• Manage Soldiers and Family member records per Army/DoD regulations.
• Facilitate warm hand-offs to program personnel during transitions.
• Collect data for planning, reporting, and program improvement, completing monthly reports.
• Implement policies and practices connecting Families to support services.
