Job summary

Patriot-Enterprises-LL is seeking a Systems Navigator (SN) for the Exceptional Family Member Program (EFMP). The role involves coordinating care for Families with special needs, performing intake interviews, documenting needs, and referring to military and civilian service providers.

The position requires home visits, collaboration with government and civilian partners, and the ability to travel. A Bachelor's degree in relevant fields and 2+ years in family-related case management are preferred.

Qualifications

• Knowledge of human service principles and family-centered care.
• Familiarity with military, state and federal laws affecting medical/educational/family services.
• Non-clinical case management skills to connect families with required care providers.

Responsibilities

• Provide home visits to Soldiers and Families to offer support.
• Coordinate with government and civilian providers for respite care resources.
• Maintain a catalog of networks and contacts for EFMP-related services.
• Coordinate monthly support groups and/or workshops.
• Present EFMP Systems Navigation briefings to Families and Soldiers.
• Provide information and referrals to Families for appropriate resources.
