Patriot Enterprises LLC in Hawaii seeks a Systems Navigator for the Exceptional Family Member Program (EFMP). You will coordinate with military and civilian providers, conduct intake interviews, and link families with essential services.

The role requires 1) strong case-management skills, 2) ability to travel, and 3) knowledge of federal regulations. A Bachelor’s in social work or related field plus experience is preferred.

Responsabilidades

• Conduct intake interviews and document family needs.
• Coordinate with agencies to locate respite care resources.
• Maintain an information catalog of networks and contacts.
• Coordinate monthly support groups or workshops.
• Present EFMP briefs to Soldiers and Families.
• Provide information and referrals to families for services.
• Manage Soldier and Family records per Army and DoD regulations.
• Facilitate warm handoffs to program personnel during transitions.
• Collect data for planning and monthly program reports.
• Implement policies linking families to support services.
