Job summary

Qualifications

• Passion for serving individuals with mental health and substance use challenges.
• Strong interpersonal, communication, and motivational interviewing skills.
• Ability to work independently while collaborating with hospitals, community partners, and multidisciplinary teams.
• Experience in behavioral health, case management, or related field preferred.

Responsibilities

• Conduct outreach and provide navigation services to individuals discharged from ED with mental health or substance use diagnoses.
• Link patients to outpatient treatment providers ensuring services within seven days of ED discharge.
• Build supportive relationships via ED contact, phone, virtual, and in-person engagement to encourage care connection.
• Assist with transportation coordination to behavioral health appointments when appropriate.
• Assess barriers such as housing stability, transportation, or food insecurity impacting access to care.
• Collaborate with hospital staff, community providers, and care teams to coordinate services and improve continuity of care.
• Engage patients during and after ED visits to support linkage to care and follow-up.
