Qualifications

Working knowledge of Medicaid and regulatory agency standards.

Responsibilities

Engages and motivates members to participate in case management programs, articulating goals and interventions.
Assists with development of family-centered care and tailored assessments for eligibility.
Facilitates communication among the healthcare team and coordinates referrals.
Provides navigation support to connect families to services and resources.
Manages transitions between settings and follows up across the continuum of care.
Answers calls, schedules appointments, and assists members with immediate needs.
