The Columbus Organization is seeking a Community Navigator for full-time care coordination in Delaware. The role supports evaluations, referrals, site visits and crisis interventions to ensure continuity and quality of care for the IDD population, with caseloads of 38-45 individuals.

Requires at least one year in health or human services and ability to work remotely within the Kent County area. Residency in Kent County is preferred; travel to multiple locations is required.

Qualifications

• Minimum one year of experience in health or human services, including interviewing and assessing needs.
• Experience contributing to client service plans and recommendations for eligibility or treatment.
• Knowledge of relevant laws, regulations, policies and procedures.

Responsabilités

• Serve as liaison for participants, families and providers to coordinate services.
• Conduct assessments and develop person-centered service plans.
• Monitor service delivery and address gaps to ensure quality care.
• Provide case management and crisis support when needed.
• Maintain accurate electronic records and HIPAA compliance.
• Build community resources and promote engagement.
• Conduct outreach to expand awareness of agency services.
