Talentify seeks a Care Coordinator for our Columbus, OH location to support a bold vision of the next generation of managed care for children with complex behavioral health needs. You will work with MCOs, vendors, and ODM to coordinate comprehensive services and drive optimal outcomes.

The role requires a bachelor’s degree or a non-licensed master level clinician with 2+ years of behavioral health, child and family services, and Medicaid experience.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with Ohio delivery systems and local community networks.

Responsabilités

• Coordinate care across MCOs, vendors and ODM to support specialized needs.
• Evaluate member needs/eligibility and determine care approach to outcomes.
• Provide care coordination, support and education using case management tools and resources.
• Serve as a single point of contact for members and remediate gaps in care.
