Job summary

Pacific Health Group is seeking an ECM Lead Care Manager (LCM) Team Lead to support frontline operations, mentor staff, and ensure high-quality, person-centered care. The role blends field-based member support with team development and adherence to Medi-Cal and ECM guidelines.

Responsibilities include coaching LCMs, monitoring workflows, coordinating care across medical and community services, and maintaining compliant documentation. Travel up to 60–70% within the county is required.

Qualifications

• 3+ years of ECM, case management, social services, or healthcare coordination experience.
• Experience mentoring, training, or supporting staff is strongly preferred.
• Strong knowledge of Medi-Cal, CalAIM, and ECM program requirements.
• Excellent communication, leadership, organizational, and problem-solving skills.
• Ability to balance member care with operational support duties.
• Strong documentation and technology proficiency.

Responsibilities

• Provide day-to-day support and guidance to Lead Care Managers (LCMs).
• Assist in monitoring team productivity, workflows, and quality standards.
• Support onboarding and ongoing development of LCMs.
• Coordinate medical, behavioral health, and community-based services.
• Maintain timely, accurate, and compliant documentation per ECM requirements.
• Assist with coverage needs and field-based member engagement.
