Job summary

Father Joe’s Villages is seeking a Supervisor of Enhanced Care Management to lead the ECM program within the Street Health Program. You will supervise ECM Care Coordinators, coordinate field operations, and ensure high-quality care planning, outreach, and crisis response to vulnerable populations.

You will monitor performance, ensure documentation quality, and collaborate with healthcare, housing programs, and partners to improve access to care and stable outcomes.

Qualifications

• Bachelor’s degree required; or equivalent combinations of education and experience.
• Minimum five (5) years of outreach, care management, case management, or related services.
• Minimum two (2) years of supervisory or lead experience.
• Experience with Medi-Cal populations and managed care strongly preferred.
• Experience using EHR, care management platforms, and data reporting systems.
• Valid California driver’s license.
• CPR certification or ability to obtain within required timeframe.
• Strong knowledge of ECM, CalAIM, Medi-Cal managed care and integrated care models.
• Ability to lead and develop care coordination teams.
• Strong communication and collaboration skills.

Responsibilities

• Lead daily operations of the ECM program within the Street Health Program.
• Supervise, coach, train, and support ECM Care Coordinators in field-based care coordination.
• Coordinate daily team workflows, workload assignments, service priorities, and staffing coverage.
• Maintain an ECM caseload and provide outreach, assessment, care planning, resource coordination, crisis intervention, and ongoing support.
• Provide guidance to staff managing complex member situations, safety concerns, barriers to care, and crisis needs.
• Monitor program performance, productivity, member engagement, documentation quality, and service delivery outcomes.
• Review documentation, conduct chart audits, and support compliance with CalAIM, Medi-Cal managed care, and organizational requirements.
• Track key program metrics and use data to identify opportunities for improvement.
• Collaborate with healthcare providers, behavioral health teams, housing programs, managed care plans, and community partners.
• Promote trauma-informed, person-centered, culturally responsive, and equity-focused care.
• Bachelor’s degree in relevant field or equivalent; minimum five years experience in related roles.
• Commitment to serving vulnerable populations with compassion and dignity.
