Job summary

Father Joe’s Villages seeks a Supervisor of Enhanced Care Management (ECM) for the Village Health Center. This community-based leadership role combines supervision, program oversight, care coordination, and operational accountability to support people facing homelessness and complex needs.

The role emphasizes coaching staff, building trust with members, and collaborating with healthcare providers, care teams, and community partners to improve access to care and housing stability.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Public Health, Human Services, Healthcare Administration, or related field required.
• Minimum five (5) years of experience providing outreach, care management, case management, or related services to vulnerable populations.
• Minimum two (2) years of supervisory or lead experience.
• Experience with Medi-Cal populations and integrated care teams preferred.
• Experience using electronic health records and data reporting systems.
• Valid California driver’s license required.
• Bilingual English/Spanish preferred.

Responsibilities

• Lead daily operations of the ECM program within the Street Health Program.
• Supervise, coach, train, and support ECM Care Coordinators in field-based care coordination.
• Coordinate daily team workflows, assignments, priorities, and staffing coverage.
• Maintain an ECM caseload; provide outreach, assessment, care planning, and resource coordination.
• Guide staff managing complex member situations, safety concerns, barriers to care, and crisis response.
• Monitor program performance data, productivity, and documentation quality.
