Job summary

Pacific Health Group is seeking an ECM Program Manager to lead our Enhanced Care Management program across California. You will supervise Lead Care Managers, ensure program quality, and drive growth while advancing health equity. Responsibilities include budgeting, cross-functional collaboration with hospitals and providers, and leveraging data to improve outcomes.

Qualifications

• Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field.
• Experience leading healthcare teams and programs (5+ years in healthcare management; 3+ years in care/case management).
• 2+ years managing Enhanced Care Management or intensive care management programs.

Responsibilities

• Lead Lead Care Managers across counties; conduct weekly 1:1s and team meetings.
• Develop and monitor program quality metrics; implement QA protocols.
• Ensure high-quality in-person member visits and documentation; monitor outcomes.
• Collaborate with hospitals, providers and community organizations to improve care transitions.
• Manage budgets and staffing; adjust resources to meet needs and priorities.
• Identify opportunities to enhance program effectiveness and pilot telehealth initiatives.
