Adecco US, Inc. is seeking an ECM Case Manager in San Jose, CA to support high‑risk Medi‑Cal members with complex medical, behavioral health, and social needs.

This community‑based position involves work in clients' homes, shelters, schools, and partner locations, with office‑based work also part of the role. The ideal candidate provides outreach, care coordination, and connection to resources, ensuring culturally competent and trauma‑informed care.

Qualifikationen

• Bachelor's degree or equivalent education, training and experience.
• At least 1 year of case management or related experience.
• Familiarity with housing and social services resources.
• Spanish, Mandarin, Vietnamese or another second language is a plus.
• Valid California driver’s license and current personal auto insurance required.

Aufgaben

• Conduct outreach and enrollment activities for CalAIM ECM program.
• Complete behavioral health, housing, and other high‑risk assessments.
• Develop and maintain individualized care plans.
• Coordinate services with multidisciplinary healthcare teams, providers, community organizations, and outside agencies.
• Facilitate referrals, resource navigation, and access to healthcare, housing, wellness, and community services.
• Help clients develop and strengthen social and community support systems.
• Advocate for clients and establish effective working relationships with community agencies and service providers.
• Maintain accurate and timely clinical documentation in accordance with applicable standards and regulations.
• Provide services in diverse community settings, including shelters, residential facilities, drop‑in centers, and client homes.
• Participate in clinical supervision, consultation, training, and team meetings.
• Provide translation or language support when appropriate.
• Perform other related duties and projects as assigned.
