Job summary

Alternative Family Services seeks an ECM Support Counselor to coordinate care for Medi-Cal members with complex needs in San Leandro and the SF Bay Area. You’ll perform outreach, enroll new clients, and deliver person-centered care management across medical, behavioral health, and social needs.

In this full-time role, you’ll work in community settings, partner with providers and organizations, and advocate for members while maintaining HIPAA-compliant records and timely documentation.

Qualifications

• Background clearances required (fingerprint, government)
• CPR/First Aid certification
• Valid CA driver’s license with good driving record
• Bachelor’s degree in social science or equivalent experience in the field
• Experience with case management and care coordination for complex needs
• Experience with outreach and public speaking
• Bilingual English/Spanish preferred

Responsibilities

• Engage community-based and healthcare organizations to generate new client-members
• Develop and participate in community outreach events to enroll clients
• Serve as member’s primary contact, delivering individualized care management
• Create person-centered care plans with members and support systems
• Meet clients in homes and communities to assess needs and remove barriers
• Navigate health and social service systems to connect to providers
• Coordinate referrals and follow up to ensure service access
• Maintain timely documentation and HIPAA-compliant records
• Communicate with health plans, providers, and care teams
• Advocate for member preferences and goals in care planning
• Participate in care team meetings and care coordination activities
