Job summary

Adecco Healthcare & Life Sciences in San Jose, CA is hiring a Case Manager – Enhanced Care Management (ECM) for an in-person, onsite role. The position is Full-Time, Direct Hire, with a typical Monday–Friday schedule and pay ranging from $25.00 to $28.50 per hour.

Responsibilities include engaging Medi-Cal members into ECM, conducting assessments, developing care plans, coordinating with providers, and advocating for clients while ensuring regulatory documentation is maintained.

Qualifications

• Bachelor's degree in a healthcare, behavioral health, social services, or related field, or equivalent combination of education and experience.
• At least one year of case management or related experience.
• Knowledge of community-based resources, including housing and social services.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work effectively with diverse populations and multidisciplinary teams.
• Valid California driver's license, reliable transportation, and current auto insurance (travel may be required).

Responsibilities

• Engage Medi-Cal members and enroll them into the CalAIM ECM program.
• Perform behavioral health, housing, and high-risk assessments to identify client needs.
• Develop, implement, and maintain individualized care plans.
• Coordinate care with multidisciplinary teams and providers.
• Provide case management, care coordination, and navigation to services.
• Help clients build social support systems and access services.
• Advocate for clients through partnerships with community organizations and providers.
• Maintain accurate clinical documentation per agency and regulatory requirements.
• Conduct outreach in shelters, residential facilities, and community locations.
• Participate in clinical supervision, team meetings, and training.
• Perform additional duties as assigned.
