Job summary

Jefferson Parish Human Services Authority (JPHSA) seeks a Service Coordinator for the EarlySteps SPOE to coordinate intake and referrals and support families through evaluation and IFSP processes. You will collaborate with families, healthcare providers, and community partners to ensure timely access to services.

The role emphasizes documentation, eligibility guidance, and connecting families to available resources in accordance with IDEA Part C and state procedures.

Qualifications

• Four years of experience in health or developmental disability services, counseling, health care management, or social services; OR
• Six years of full-time work experience plus one year of related experience; OR
• Bachelor's degree plus one year of related experience; OR
• Advanced degree in social sciences, nursing, public health, health administration, or related field.

Responsibilities

• Serve as the initial point of contact by promptly receiving, reviewing, and processing referrals for EarlySteps and other programs.
• Enter, track, and maintain referral information in electronic systems within program timeframes.
• Establish a trusting, family-centered relationship respecting the family's culture and priorities.
• Conduct preliminary intake interviews to gather developmental concerns, needs, and supports.
• Complete intake questionnaires to identify potential service needs requiring further assessment.
• Provide information and guidance about developmental disability programs, eligibility, and resources.
