Job summary

Care Lync is seeking a Supports Coordinator for Early Intervention Services to coordinate services for children birth to age 6 and their families. You will help families access needed services, understand rights, and navigate multi-agency support with a focus on family empowerment.

The role involves collaboration with a multidisciplinary team, documentation, and travel for community-based service delivery. A Bachelor’s degree in a relevant field and at least 1 year of experience is required.

Qualifications

  • Bachelor’s degree in one of the listed fields or related area.
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

  • Serve as primary point of contact for families and coordinate services.
  • Assess and plan via IFSP/Family planning and multidisciplinary meetings.
  • Educate families on rights, safeguards, and advocacy.
  • Monitor service delivery and coordinate across systems.
  • Maintain accurate documentation and ensure compliance with Medicaid and IDEA Part C.
  • Collaborate with families, providers, and agencies to support transitions.

Education

Bachelor's degree in Social Work
