Job summary

Jimmy Jazz in Roswell, NM seeks a Family Service Coordinator to help families of children birth to 3 access and receive early intervention services. The FSC develops the IFSP, coordinates services, and supports families in navigating resources across home and community environments.

The role involves coordinating evaluations, ensuring timely delivery of services, and maintaining regular family meetings to monitor progress and plan transitions as needed.

Responsibilities

• Explain to families the early intervention process, parent rights and safeguards
• Gather information from the family regarding their concerns, priorities and resources
• Coordinate the evaluation and assessment activities
• Facilitate the determination of the child’s eligibility
• Connect the family with resources and supports
• Coordinate with medical and health providers
• Help families plan and prepare for their IFSP
• Organize and facilitate monthly and periodic meetings with the family and other team members
• Arrange and coordinate all services listed on the IFSP
• Monitor the delivery of services
• Facilitate periodic reviews of the IFSP and ensure a transition plan is developed at the appropriate time and transition conferences are held
• Provide families with information and support to enhance their child’s development in all areas (cognitive, communication, physical/motor, social or emotional, adaptive development)
• Complete the Public and Private Insurance form with each family to determine eligibility for Medicaid or private insurance
• Meet with the family at least monthly through face-to-face visits
• Manage and coordinate the transition process beginning with the transition plan and finalization at the transition conference
