Responsibilities

• Explain to families the early intervention process, parent rights and safeguards
• Gather information from the family regarding their concerns, priorities and resources
• Coordinate the evaluation and assessment activities
• Facilitate the determination of the child’s eligibility
• Connect the family with resources and supports
• Coordinate with medical and health providers
• Help families plan and prepare for their IFSP
• Organize and facilitate monthly and periodic meetings with the family and other team members
• Arrange and coordinate all services listed on the IFSP
• Monitor the delivery of services
• Facilitate periodic reviews of the IFSP and ensure a transition plan is developed at the appropriate time and transition conferences are held
• Provide families with information and support to enhance their child’s development in all areas (cognitive, communication, physical/motor, social or emotional, adaptive development)
• Complete the Public and Private Insurance form with each family to determine eligibility for Medicaid or private insurance
• Meet with the family at least monthly through face-to-face visits
• Manage and coordinate the transition process beginning with the transition plan and finalization at the transition conference

Description

We are currently looking for a Family Service Coordinator who wants to help families in need of support. Family Service Coordination services are carried out by a designated individual to assist and enable the families of children from birth to 3, to access, and if determined eligible, receive early intervention services. The Family Service Coordinator helps to develop the Individualized Family Service Plan (IFSP); assists the family in receiving all services identified; coordinates those services; ensures that they are delivered in a timely manner, and seeks additional services and or supports that may help the child or family. These services will be conducted within a variety of environments in home and community settings.

ESSENTIAL DUTIES

The FSC will operate within Local, State and Federal Regulations as well as adhere to all Tobosa policies and procedures. Essential duties include but are not limited to the following:

• Complete the Public and Private Insurance form with each family as applicable at least annually to determine if the child is or may be eligible for Medicaid or if they are covered under a private insurance plan


• Meet with the family once a month at a minimum through a face-to-face visit


• Manage and coordinate the transition process beginning with the transition plan and finalized at the transition conference
